How We Can Help:
The Planned Care Service facilitates long term packages of care for adults enabling people to live independently in their own home in the community.
The Community Planned Care Team will visit at agreed times throughout the day to support with Personal Care;, assistance to bathe and dress, administer medication, support activities and appointments and provide practical support within the home.
