Sunderland Care and Support operate a Community Reablement Service as a part of a city wide Reablement at Home Service.

The service provides vulnerable people and their carers with high quality personal care, assistance and support; helping them to live as independently as possible at home through;

• Positive support, assisting people to regain skills they may have lost due to decline in health
• Building confidence and supporting to reach set goals
• Helping people to manage daily tasks themselves, maintaining independence
• Supporting a therapy plan

The Reablement Service is a time-limited service and can be accessed via a referral from a hospital, GP, or therapist.
