Farmborough Court Intermediate Care Centre provides short-term, person-centred rehabilitation and support for individuals who are transitioning from a hospital stay back to their own home or into another care setting. Our focus is on supporting each person's recovery, restoring independence, and helping customers return to daily life with confidence while reducing the risk of unnecessary hospital readmission.

At Farmborough Court, we recognise that every individual's journey is unique. We provide a supportive, structured, and compassionate environment where people can recover at their own pace, with care and support tailored to their specific needs, goals, and circumstances.

Following a period of illness or hospitalisation, individuals are supported through a personalised programme of rehabilitation. This may include input from physiotherapy, occupational therapy, and where appropriate, speech and language therapy, alongside ongoing clinical oversight. Our approach is designed to help individuals regain strength, improve mobility, rebuild skills, and increase confidence in managing everyday activities.

Stays at Farmborough Court are time-limited with a clear focus on enabling individuals to achieve the highest possible level of independence. During this time, we work closely with each person to identify what matters most to them and to support their progress towards returning home or moving to a more independent living arrangement.

In addition to therapy and rehabilitation, we provide support with goal planning including support in building confidence with personal care, medication management, and daily living activities. We also encourage social engagement and wellbeing, recognising the importance of emotional and social health as part of recovery. Our aim is to provide holistic care that supports the whole person, not just their physical needs.

Farmborough Court operates as part of a multidisciplinary team, including nurses, therapists, social workers, and medical professionals. We work collaboratively with individuals, their families, and wider support networks to develop and deliver personalised individual goal plans. We also liaise closely with community services and Adult Social Care to ensure a smooth and coordinated transition once individuals leave the service.

By focusing on what individuals can achieve, rather than what they cannot, Farmborough Court plays a vital role in supporting recovery, rebuilding confidence, and promoting independence. We are committed to helping people regain control of their lives and maintain their dignity, choice, and quality of life. Referrals to Farmborough Court can be made by a GP, social worker, nurse, or therapist, either from hospital or within the community.
