Sunderland Care and Support's Reablement Service provides specialist, short-term support to individuals who have experienced a decline in their physical abilities. The primary aim of the service is to enable individuals to regain independence, improve their overall wellbeing, and continue to live safely within their own homes or communities.
As part of a multidisciplinary approach, SCAS works collaboratively with a range of professionals, including occupational therapists, physiotherapists, and social workers. This ensures that individuals receive a comprehensive assessment, which informs the development of a personalised and goal-focused reablement plan.
The service is centred on supporting individuals to rebuild confidence and capability in undertaking activities of daily living, such as personal care, dressing, meal preparation, and managing household tasks. In addition, support may extend to improving mobility, communication, cognitive functioning, and emotional wellbeing.
The Reablement Service is delivered as a time-limited and intensive intervention, during which period, the team works alongside individuals to develop skills, promote independence, and maximise functional ability. The overarching aim is to support individuals to achieve their highest possible level of independence while reducing the need for ongoing care and support.
Reablement is delivered in close partnership with individuals, their families, and other relevant professionals to ensure a coordinated and holistic approach. By promoting self-reliance and empowering individuals, the service plays a key role in maintaining dignity, supporting social inclusion, and enhancing overall quality of life.
